In the instance that they don’t have your record, you can file for a Delayed Registration of Birth or better yet, go back to the Local Civil Registrar of the hometown where you were born to obtain a birth certificate from them.

There isn’t much requirements needed for you to get your NSO birth certificate except for the municipal birth certificate that you’re holding. And once you have that, you can go to any branch of NSO near your place. (On a personal note, Manila branches are way better for me.)
Once you are in the office, you will be asked to fill out an application form. Therefore, it is important that you know some necessary information such as the name at birth, birthday, birthplace, your parents’ name as well as you mom’s maiden name, and the year of birth registration.
Now, the last thing to do is to fall in line, and wait for your application to be processed. This entails payment fee of 125 Php wait for its release and tada – you have it in one day. Its validity doesn’t expire. In cases of correction and change name, you may have to send docs supplementing the correction via mail to the NSO main office in Quezon City.

Applying for the registration of birth in person may be a hassle since you will have to fall in line for hours. Well, if this is not your thing, the office is now offering online application. To do this, you can go to their website by clicking the National Statistics Office link.

Get in online and have it delivered in 3-4 days. Payment may be made through credit card and other bank transactions. The cost of online application may be a little higher than how much you pay when visiting the office. The rate includes delivery price of the document. Roughly, the amount to be paid is about 300 Php.

In addition, the company also offers passport processing, marriage certificate, CENOMAR (singleness), and death certificate which may be processed online and in a more efficient way. Other inquiry may be done by calling their hotline 737-1000.
